摘要：关于基本医疗保险对主观幸福感的影响,以往文献忽略了医疗服务价格上涨这一因素,从而可能高估了医疗保险的效果。基于此,使用CGSS 2005年和2010年的两期截面数据,采用IV-Probit方法,系统探讨了基本医疗保险对主观幸福感的影响。在控制了医疗服务价格这一关键变量之后,全样本中医疗保险的补贴效应占了主导地位,进一步收入分组后则发现明显的异质性,在较高与最高收入阶层以补贴效应为主,但在农村最低收入阶层以挤出效应为主,原因可能在于医疗服务价格的上涨,致使农村最低收入阶层就医时仍具较高家庭负担。应针对农村最低收入人群灵活设计"靶向扶持"的差异化政策,对其进行精准的健康扶贫,避免再次进入"低收入陷阱"。
About the impact of basic health insurance on happiness,the previous literature neglected the factor of rising medical service prices,which may overestimate the effect of health insurance.The article uses CGSS data of 2005 and 2010 to evaluate the impact of health insurance on happiness with IV-Probit method.It is found that after controlling the key variable of medical service price,the subsidy effect of medical insurance dominates in the full sample,and there is obvious heterogeneity after further income grouping.The subsidy effect dominates in higher-income and highest-income group,but the crowding-out effect dominates in lowest income group from rural areas.The reason may lie in the rising price of medical services,which makes the rural lowest-income people still have a higher family burden when they seek medical treatment.Therefore,this paper proposes that in order to avoid them re-enter the"low-income trap",the differential policy of"targeted support"should be flexibly designed for the rural lowest-income group,and conduct the targeted health poverty alleviation.
