摘要:目的探讨上海市7~12岁儿童在超重和肥胖层面的腰围界值点,为制定上海市7~12岁儿童健康腰围标准提供参考。方法以上海市2 685名7~12岁小学生为研究对象,测量其身高、体重、腰围和腹部皮褶厚度等指标。采用最小均方(LMS)[12]算法,按照性别和年龄分别作出P5、P10、P15……P95腰围百分位的平滑曲线。以BMI为基准,通过ROC曲线分析上海市7~12岁儿童腰围在超重和肥胖层面的界值点。采用偏相关分析(控制变量为身高)探讨腰围、BMI与腹部皮褶厚度之间的相互关系。结果上海市7~12岁年龄段男生、女生的腰围百分位数值均随年龄的增长而增加。上海市7~12岁年龄段男生腰围各百分位数值有高于全国同龄男生的趋势;上海市7~9岁年龄段女生腰围各百分位数值与全国同龄女生基本持平,上海市10~12岁年龄段女生腰围各百分位数值高于全国同龄女生。在超重层面,男生腰围界值点主要集中于P65(58.12~71.62cm),灵敏度和特异度分别为0.825~0.942和0.824~0.925;女生腰围界值点主要集中于P80(57.49~69.74cm),灵敏度和特异度分别为0.788~0.950和0.881~0.934。在肥胖层面,男生腰围界值点主要集中于P80(61.27~76.80cm),灵敏度和特异度分别为0.713~0.960和0.816~0.940;女生腰围界值点主要集中于P90(60.73~73.98cm),灵敏度和特异度分别为0.872~0.998和0.874~0.956。在控制身高变量后,上海市7~12岁儿童腰围与BMI(男、女生偏相关系数分别为0.822、0.740)、腰围与腹部皮褶厚度(男、女生偏相关系数分别为0.726、0.656)、BMI与腹部皮褶厚度(男、女生偏相关系数分别为0.718、0.604)间均呈正相关(P值均<0.05)。除8岁男生、11岁男生和8岁女生外,其余上海市各年龄男生和女生的腰围与腹部皮褶厚度间偏相关系数均大于BMI与腹部皮褶厚度间偏相关系数。上海市各年龄男生腰围与BMI、腰围与腹部皮褶厚度、BMI与腹部皮褶厚度间偏相关系数均大于同龄女生。结论上海市7~12岁儿童腰围界值点在超重层面上,男生为P65(58.12~71.62cm),女生为P80(57.49~69.74cm);在肥胖层面上,男生为P80(61.27~76.80cm),女生为P90(60.73~73.98cm)。
Objective To investigate the cutoff point of waist circumference (WC) for overweight and obesity, so as to provide a reference for the development of 7 - 12 years school-age children in Shanghai WC criteria. Methods The height, weight, WC and abdominal skinfold thickness of 2 685 primary school students aged 7- 12 years in Shanghai were measured. The percentile curve of WO was made through least mean squares (LMS) method. Taking the body mass index(BMI) as criteria we analyzed, the cutoff points of WC for overweight and obesity by receiver operator characteristic (ROC) curve. The relationship of WC, BMI and abdominal skinfold thickness was analyzed by partial correlation. Results The percentiles of schoolboys and girls aged 7- 12 years in Shanghai were increased with age, and were higher than those of schoolboys and girls of the same age in the country. The percentiles of schoolboys aged 7- 12 years in Shanghai were higher than those of schoolboys of the same age in the country. The percentiles of schoolgirls aged 7-- 9 years in Shanghai were almost equal to those ofschoolgirls of the same age in the country, and those of schoolgirls aged 10- 12 years in Shanghai were higher than those of the same age in the country. For overweight, WC cutoff point of boys among 7- 12 years old in Shanghai was mainly at the P65 (58. 12 - 71.62 cm), with the sensitivities and specificities being 0. 825 -- 0. 942 and 0. 824 -- 0.925, respectively; and the WC cutoff point of girls was mainly at the P80 (57. 49- 69. 74 cm), with the sensitivities and specificities being 0. 788 - 0. 950 and 0.881 - 0. 934, respectively. For obesity, the WC cutoff point of boys among 7- 12 years old in Shanghai was mainly at the P80 (61.27-76.80 cm), with the sensitivities and specificities being 0. 713 - 0. 960 and 0. 816 - 0. 940, respectively; and the WC cutoff point of girls was mainly at the P90 (60.73 - 73.98 cm), with the sensitivities and specificities being 0. 872 -- 0. 998 and 0. 874 -- 0. 956, respectively. After controlling the height variable, the partial correlation coefficients between BMI and WC were 0. 822 in 7- 12 years old boys and 0. 740 in 7 - 12 years old girls, those between WC and abdominal skinfold thickness were 0. 726 and 0. 656, and those between BMI and abdominal skinfold thickness were 0.718 and 0. 604 (all P〈0.05). The partial correlation coefficients of WC and abdominal skinfold thickness were higher than those of BMI and abdominal skinfold thickness in schoolboys and schoolgirls at each age, respectively, except for 8 years old boys, 11 years old boys and 8 years old girls. Conclusion The WC cutoff point for Shanghai school children is P65 (58.12- 71.62 cm) in boys and P80 (57.49 - 69.74 cm) in girls for overweight, and was P80 (61.27 - 76.80 cm) in boys and P90 (60.73 - 73.98 cm) in girls for obesity.
