摘要:ObjectiveTo systematically evaluate the overall effects of aerobic exercise on anxiety among university students and to compare the relative effectiveness of different aerobic exercise modalities.MethodsRandomized controlled trials (RCTs) published from database inception to January 2026 were retrieved from Web of Science, PubMed, Scopus, Embase, the Cochrane Library, SPORTDiscus, and China National Knowledge Infrastructure (CNKI). Eligible participants were university students receiving aerobic exercise interventions compared with no-intervention controls. Risk of bias was assessed using the Cochrane RoB 2 tool. PMA was first conducted to estimate the overall effect of aerobic exercise on anxiety. Subsequently, NMA was performed to compare different aerobic exercise modalities and to rank them using the surface under the cumulative ranking curve (SUCRA). The certainty of evidence was assessed using the CINeMA framework.ResultsTwenty RCTs involving 1625 participants were included. The PMA showed that aerobic exercise significantly reduced anxiety among university students (SMD = -0.73, 95% CI: -1.01 to -0.45). Subgroup analyses indicated that exercise frequency significantly moderated intervention effects (p = 0.004), with programs performed three times per week showing the greatest reduction in anxiety (SMD = -0.87, 95% CI: -1.27 to -0.46). Sensitivity analyses confirmed the robustness of the findings, and Egger's test indicated no significant publication bias (p = 0.333). The NMA suggested that multicomponent aerobic exercise had the highest probability of being among the most effective interventions (SUCRA = 95.3%), followed by mind-body exercise (61.4%) and conventional aerobic exercise (38.3%). Network meta-regression showed that publication year, sample size, intervention duration, exercise frequency, and session duration did not significantly moderate treatment effects.ConclusionAerobic exercise significantly reduces anxiety among university students. Programs conducted approximately three times per week may provide favorable outcomes. Network meta-analysis further suggested that multicomponent aerobic exercise and mind-body exercise had higher probabilities of being among the most effective modalities, although the certainty of evidence was very low. Further high-quality RCTs with larger samples, direct comparisons of exercise modalities, and longer follow-up periods are needed to clarify optimal exercise strategies for anxiety reduction in this population.
