摘要：在不同疾病阶段,老年人医养需求具有不同的结构和特点。为集约资源利用,提升老年人的健康照料水平,需要由功能定位不同的医疗机构、护理院、养老院、社区、家庭等为处于生命周期不同阶段的老人提供差异化的服务。医养融合应以社区为平台打造智慧网,整合上述资源,为老人提供全过程的有针对性的服务,整体解决老人的医疗和照护需求。医养融合面临一系列内生性矛盾,要破解这些矛盾,需要策略性的制度嵌入,主要包括社区医护增能赋权、完善资源整合配置机制和发展护理保障实现老年全面健康保障等。
In the different stages of disease,the medical and care needs of the elderly with different structure and characteristics. To make rational utilization of the resources, it is necessary to make the medical institutions, nursing homes, communities, and families to provide differentiated services for elderly in different phases of the life-cycle. Program of all-inclusive medical and care should take the community as a platform and construct an intelligent network to integrate the resources,and provide focused services for the whole process of the elderly,and take a total solution to the medical and care needs of the elderly. Facing with a series of endogenous contradiction,the program of all-inclusive medical and care need embed strategic system,including the empowerment of the community medical care,the integration and allocation mechanism of the resources and the development of long term care security system.
