摘要：如何实现养老主体的耦合并提升养老服务质量是当前养老主体多元化面对的主要问题。长护险实践表明,在地化的医养结合能够形成准公共性养老秩序,实现多元主体的耦合。长护险的制度设置可以筛选出有需要的服务群体,进而以在社区内选拔护理员的方式激活社区内部的养老服务供给力量。通过在地化实践,市场型养老服务的契约关系嵌入到熟人关系体制中,充分激发了熟人社会内部的互助网络。服务主体内生化、服务内容面向需求以及服务监管的非规则化构成多元主体互惠耦合。互惠模式的养老秩序之实现离不开以长护险为形式的医养结合在地化实践、家社互构的养老角色以及制度的输入与资源的扶持。研究表明,互惠型医养秩序的形成,能够超越社区养老中的市场、社会与行政之争,实现多元主体的有效结合。
How to realize the coupling of the subjects of old-age care and improve the service quality of old-age care is the main problem that the subjects of old-age care face at present.The practice of Suzhou Mayor's insurance policy shows that the combination of local medical care and nursing care can form a quasi-public order of old-age care and realize the mutual coupling of multiple subjects.The Long-term Care Insurance System can screen out the need of service objects,and then select nurses in the community to activate the supply of old-age services in the community.Through the localization practice of the old-age provision,the contractual relationship of the market-based old-age service is embedded in the acquaintance relationship system.The endogenization of the service subject,the demand-oriented of service contents and the irregularity of service supervision constitute the mutual coupling of multi-subjects.The realization of the old-age order of the mutual benefit model cannot be separated from the practice of medical care in the form of long-term insurance,the role of family and society in old-age care as well as the institutional support and resource-based support.The formation of the order of reciprocal medical care goes beyond the dispute among the market,society and administrative subjects in community care for the aged to realize the effective combination of multiple subjects.
